The sample municipal reimbursement form linked to above is for informational purposes only. Municipalities should reference this form when tracking expenses. However, this form may not be used to submit a reimbursement request. All applications for municipal reimbursements must be submitted online through the Pennsylvania Electonic Single Application (ESA) website.
The Municipal Reimbursement Form Instructions document should be references by municipalities for guidance on determines costs that are eligible and ineligible for reimbursement.
